I've ranted about using Notary Publics or other sorts of things for years, usually in the context of pointing out that the US Passport Office does not require everybody to go to New Orleans to get a passport.  Never put it down on paper so succinctly, though.

Seems like a perfectly elegant solution to me..  Although it will likely not be fast enough for our modern world, where we have simultaneous requests for increased security of identity and authentication (which takes time to do right) and demand for just-in-time credentialing.

Another (possibly perceived as "safer" -- although I think that can be argued) method for delivering the short-lived token would be for the paper form to also assert control of a phone capable of text messaging, and send the token as a text message (or, part via email and part via text).
