Nice survey. A couple of comments:

Do we want to know what role the campus might be willing to play (and maybe 
you folks talked about this already)? For example, would they be a lead 
campus? An initial tester with the provider? Just sign the letter? 

You probably know this, but there's logic in survey monkey that enables you 
to add another question on-the-fly based on their Yes/No clicks. Using this, 
you won't have to ask them to fill out #3 based on #2, the tool will just 
provide them with the right question. The way you have it is nice for 
reviewing the entire survey, but didn't know if you want to include this 
feature. (Picky point!)

Do we care if several people from an institution complete the survey and 
possibly provide conflicting answers? 

I assume this will be send to the InC Participants as well as the InC Library 
list. Are there others?

Should we provide a link to the InC Library page for background and/or a link 
to InCommon?
