InCommon metadata support

In conjunction with the InCommon Per-Entity Metadata Pilot, [1] the
beta instance of the Metadata Query Server running at
mdq-beta.incommon.org now serves *both* per-IdP metadata *and* per-SP
metadata. The latter is usable with Shibboleth IdP 3.0, which was
released on December 22, 2014. [2]

The Metadata Query Server at mdq-beta.incommon.org sources SAML
metadata from both InCommon metadata and eduGAIN metadata. Currently
there are more than 1500 IdP entity descriptors and 3100 SP entity
descriptors served from mdq-beta.incommon.org.
