I found a useful review of survey tools from U. Missouri Columbia and Penn 
State (attached)

UW System has a site license for the Qualtrics survey tool 
(http://www.qualtrics.com/). It looks great.  I think this would be 
considered acceptable use, especially since Karen Hanson is interested in the 
results.

Qualtrics does all the cool survey branching, question style things we want 
for the survey.  Worth a look, I think.

I will miss the call this afternoon, sorry.
